How many inches does it add? will I need to cut any holes? How much would a professional charge to install this?

it's purely ornamental and adds the illusion of length and girth. You do not need to cut any holes, just insert into zipper area after you put your pants on and before you zip up your pants (this also makes it harder to get your you-know-what caught in the zipper). Professional installation can be arranged for an additional $150 and it comes with complementary bath, massage and happy ending.
