Kinda similar but a little different.
Long time ago I witnessed a crew chief filling one of the front landing gear wheels for a UH60 Blackhawk up with nitrogen. Now when one is flat you are suppose to remove wheel, take to a wheel cage and inflate it there. Never on the bird.
Maybe she was distracted or for some unknown reason who knows but she inflated it while on the bird to the point that the tire unseated with enough pressure that it tore in two. The out side piece caught her left leg just below the knee and shattered both tibia & fibula bones, compound fracture. She was lucky to keep her leg.
Accident investigators couldn't confirm wither the pressure gauge was off, emergency cut off valve was stuck or she misused to equipment.
Either way she spent a yr learning how to walk again.

Some people just don't think before they act. Like the guy at work putting a personal radio (plugged into the wall) in an open aircraft fuel cell. Not smart. The guys here were lucky alright.
