All? A 78 Z will already have front disc.
The rear is easy to adapt. They used the 8.5" 10 bolts with backing plates that were standard among all the GM intermediates. Some people grabbed rear disc brake setups from cadillacs with rear disc (same backing plate bolt pattern) and swap to a vette master cylinder but the junk yards have dried up of most of those good scores.
Easiest way to do it now is to buy the conversion kit. Many suppliers like Master Power Brakes. I'm sure if you google you'll find a ton of suppliers, the aftermarket is flooded with rear disc brake kits for any GM rearend.

^^ This. A kit is probably the easiest way to go and there are lots of suppliers to choose from. Not sure, but you may need to utilize a different proportioning valve with rear discs.

Yep Jeff is right, different proportion valve. It all comes in kit form now and makes things easy.
