rebuild 383 or stock
is there any significant improvement in horsepower with heads and cam, that would make it worth while to do a 383 with an 99 ls1.

yup and 383 will give you more torque as well as hp but more so torque. You can get good hp out of a stock bottom end but up to a certian point.

You just need to come up with a plan. Say I want to drive it every day and make 400 hp at the rear wheels. You can do that on a stock bottom end and a reworked head and cam, as long as you have some supporting mods like CAI, intake, exhaust, ect. If your state has inspections what do they inspect and how. This plays a big roll in a street car. Are you going to race it and if so how much and what type of racing? then you build the motor off of that and a long term plan. Do you want to run NOS or boost. got figure that stuff out.
