First item on this page, actually comes with 3 bolts and two spacers for the front and reuses rubber radiator mounts (the two with the points sticking up) from stock support on top of radiator.

Never installed.
Has screen, 26 of the holes are slightly bent from my careless storage. Screen is still very usable or you can take it off like everyone else.
