Is it worth it?? Please let me Know!!!
I have a buddy who has a Lunati cam he is taking out of his truck. First off will it work and secondly if it will is it worth it. The specs on the cam are 221/221 @ 50 dur 558/558 lift. Would this cam be worth changing for performance and if so what would you think it would cost to change. Also what all would I need. My car has a whisper lid, smooth mass air halves, Flowmaster catback, spec-3 clutch and 4.10 gears.

Well it is an LS1 cam he did a motor swap. He pulled out the 4.8 and went with an LS1.

Cam swaps are easier on Gen III/IVs than most other engines, you don't even have to take the intake manifold off. There's a mechanism that can lock all of the pushrods in their upmost position. Take out the old, install the new, degree it and release the locks.
I'll find a link if I can, but a quick google search should bring it up.
$150 for a cam isn't too bad. Lunatis usually go for about 350.

It's a used cam, but I just wonder will I notice a difference in performance.

What's the LSA on that cam? I used to have a Lunati 221/558/114 (MTI B1 cam) that made 385rwhp with bolt-ons in my SS.

The cam has a 221/221@50 duration 558/558 lift with 114 lobe sep. Will I have to change the lifters or will they be fine. The cam has about 15,000 miles on it and my car has about 90,000. If I don't have to change lifters then all I will need is valve spring. How much are those and what else will I need to do this? How about tuning. I heard the cam and it sounds good for my basically stock car.

Tuning is always something you would want with a cam. I didn't get mine tuned, but only because I had some other things going as well. Otherwise I would have tuned it.
You will need valvesprings. I recommend Comp 918's for that cam. Lifters should be okay. As long as they are good now. I would also recommend an LS6 ported oil pump and an LS2 timing chain. You will be there, and with the mileage on your car, they are insurance. Plus, the oil pump is overall better.

What kind of horsepower will that produce? Is it worth spending the money to do this with a used cam?

I would say with tune that cam would be good for 30 RWHP or so. Maybe a little more.
There are many other options out there for cams. Gonna cost a little more, but you will get more power outta them.

What do you recommend with a cam. I love the lope at idle. That cam has a decent lope at idle but I really like a radical lope. I plan to install headers, ory, loudmouth, and ls6 intake. I want something to work with that.

First off, don't go with the Loudmouth as your catback with an ORY and headers. You will hate it. Trust me. I cut mine off. Magnaflow sounds good and flows really well for the price.
Cam recommendations depend on what you want. Just the sound?? Or the power with it? You plan on boosting or spraying the car?? Where do you want the power? Do you have to pass smog? Guessing not by the ORY.

I would say something along 224/224 59x/59x 113.
Maybe a little more duration.
