intermittant rough idle, off idle hesitation

I'm not sure where to start with this one. I'm having issues with intermittent rough idle and off idle hesitation/stumbling. Sometimes when sitting at a light the idle will get rough and low, then when I take off the car hesitates or stumbles then goes. Either problem can happen by itself, like an ok idle, but then it stumbles, or at the same time. It only hesitates for a second or 2, then runs fine.

I haven't noticed anything at freeway speed either.

It has stalled on my a few times though, only at low speed though, like if it starts acting up in a drivethru or when backing out, the low idle and hesitation leads to a stall since I am only feathering the pedal to move.

Codes pulled are P0171, P0174, and P0418. I've checked fuel pressure and its within spec. No misfire codes seen yet. MAF has been cleaned.

I've had a problem with the TPS before, and replaced it about a year and half ago. I was thinking maybe its bad again since the problem is only at very low throttle opening. O2 sensors need to be replaced, I haven't had the money or time to do them yet.

Any one have have an idea or a direction I should look in?

throttle body need cleaned ? an o2 sensor makes sense cuz they tell the computer how much fuel to feed the engine. i had a similair problem but replaced an o2 and plugs.
