At my place it is 5 incidents and you are wrote up. You lose an incident six months from the day it occurred. But as far as writeups...my friend has 14 writeups from the 8 years he has been there and is still fired, at my place they use it as an excuse to fire someone if they want to. It is no guarentee of being fired.
