There's a more complete video on youtube with commentary from the shooter. Earlier during his shooting session, he was shooting a striker fired pistol drawn from a L3 Serpa (thumbreak + trigger guard retention) with much longer takeup. He transitioned to a 1911 also from a L3 Serpa and during the draw fumbled the thumbreak and swept the thumb safety. He left his trigger finger hanging after disengaging the trigger guard retention mechanism.
This was 100% a negligent discharge, but the bottom line still, friends don't let friends draw from a Blackhawk! Serpa. It's a ridiculously stupid method of retention that doesn't accomplish anything a SafariLand ALS won't do without putting your nose picker near the trigger.

I have the blackhawk Serpa and drawl from it with no problems at all. I also carry a Glock and it has no traditional safety. It's how you train. It is so ingrained into my head to never touch the trigger unless I'm going to shoot. PPl forget that the #1 Safety feature on any firearm is not having your finger on the trigger.
His problem was that he changed pistols. He got into a rhythm from the previous pistol that when he switch he was still in the habit. When you change pistols you need go through the motions again to get use to that weapon because we all know that pistols are very different in their actions (trigger pressure, magazine loading, safety, hammer/no hammer, revolver/semi).

Just because it can be trained around doesn't mean it's not a shitty design. If you were to go to a force on force CQB course with simunitions I guarantee you'd see simulated ND's with shooters using Serpa's more than any other retention holster combined. Thumb break retention releases are far superior from a safety standpoint.
