Throw the bolt away and buy new bolts. Make sure you get the right size and the right threads. If you have loctite they are pulling thru. It seems strange ALL are pulling out on you, I have a feeling its a bolt problem.

I have a fast 92 and have no issues with backing off

What kind of vacumm are you seing? If the intake is not seated properly, you will have a good leak and the idle could possibly fluctuate. I hope they didn't tune it this way.

No vacuum leaks. The bolts have not gotten that loose yet.
