I've seen it happen a few times. It usually screws up the threads on it's way out.

Like said above, you need to pick up a thread chaser or a tap & die set and check the threads on the two cylinders and go through and check the torque of all your spark plugs.
    Which plugs are you using?

5.4 liter fords are known to do that. The original plugs only had threads half way up the plug instead of all the way and because of that, they have a habit of blowing right out of the hole and taking the threads and the coil out with it. Also have personally seen the center part of the porcelain blown out leaving the metal half of the plug still in the hole, never did figure that one out. Cant say that I have heard of this prob on a ls block though. Sorry for the thread jack.

I've seen it happen on an H22 in my friend's car. It blew a hole completely all the way through the hood. He used heli coil as a temporary fix before getting a new ported head but the heli coil seemed to hold up fine.

I've had it happen on a customers car before. Blew 2 of them out. Hers was attributed to water in gas. She sued and actually won that one.
