If you're looking to wield a knife and a handgun at the same time, I always thought the way Snake held his gun/kabar in Metal Gear Solid 3 was really cool:

These pics are pretty dark and crappy, but he holds it downward along the grip.

In one the of the cut-scenes in the game, it even shows him cutting away at the grip of a newly acquired handgun to fit the handle of the knife, while explaining to his female ally that it allows him to maintain control of the handgun while still having the knife ready for close-quarters combat.

Although the concept of a knife attached to a handgun goes back to the black powder days, I thought this modern handgun bayonet was equally retarded.

Perhaps a bayonet on both the assault rifle and the handgun would be worthy of a youtube video on Ninja Weapons!!
